You can schedule an appointment by calling our clinic, using our online booking system, or walking in for urgent care needs.

Please bring a government-issued photo ID, your insurance card, a list of current medications, any relevant medical records or test results, and your immunization records. If you were referred by another doctor, bring the referral letter and contact details. Also, have a method of payment and a list of questions or concerns.

To request a prescription refill, you can call our office, or visit the clinic in person. Please have your prescription details and pharmacy contact information ready. For some medications, a follow-up appointment may be necessary before the refill can be approved.
